**From CCPH Events page regarding 2016 Annual Conference survey

For over 30 years, CCPHers have come to expect and look forward to our Annual Conference. In addition to offering professional development opportunities, the CCPH conference is regarded by many as a reunion of friends, colleagues and mentors.

Our CCPH 2016 Annual Conference was held at California State University, Sacramento. We appreciate the efforts and participation of all those who attended and we hope to see you again at our next conference.

CCPH would very much appreciate the participation of CCPH members and conference attendees in a five minute survey to let us know your thoughts regarding the value of our organization as well as your opinions concerning the most recent conference. For those who were unable to attend the conference, please feel free to skip those specific questions in the latter half of the survey. The CCPH Board thanks you again for your input and consideration. Please complete the survey here.
